 The Care Team’s primary goal is to enable individuals to receive  person centred care and support in the comfort of their own homes. This type of care aims to help individuals maintain their independence, autonomy, and quality of life while receiving the assistance they need to thrive. By providing a range of services, such as assistance with daily activities, medical care, and emotional support, home care helps individuals to stay in familiar surroundings and maintain their connections to their community and loved ones. Ultimately, the goal of home care is to promote the well-being and overall health of individuals, enhance their quality of life, and support them in living as independently as possible.

How it works

The Care Team will conduct a comprehensive tailored assessment before providing services to a client. This assessment involves gathering information about the individual’s medical history, current health condition, medications, and any specific care needs or preferences. Our Client Assessment Co-ordinator will visit the client’s home to evaluate their living environment, safety considerations, and any potential risks. They will also assess the client’s ability to perform daily activities and identify areas where assistance may be required. Based on this assessment, a customized care plan is developed that outlines the specific services and support needed to meet the client’s unique needs and goals. This assessment process ensures that the care provided is tailored to the individual’s requirements and promotes their overall well-being and independence.
