Convalescent / Respite Care
The Care Team have links with high quality convalescent centres and can communicate with them on your behalf. Clients recovering from surgery or illness can leave the hospital but may not feel confident to be at home without support. It is important that you have the right support in place when you are being discharged from hospital. The first four weeks post-surgery or illness are can be a crucial time in the recovery process.
Having a comprehensive home care package can enhance you rehabilitation. Whether you have had surgery or need time to rebuild your confidence after an illness The Care Team will be there to help you return to strength.
The Care Team will develop a recovery care plan to suit each client. A specialised home care package led by nurses with support from professionals will be implemented.
