The role is fulltime position and will be part of our managment team and involves the following:

  • Case management – identifying needs and requirements for services users, providing assistance in achieving goals for service users to remain in the community.
  • Setting up care packages for both HSE and private clients.
  • Managing a team of supervisors and healthcare assistants in the community.
  • Partake in MDT meetings both in the hospitals and community settings.
  • Applicants must have Bachelor Degree in Nursing or Social Care or in any other healthcare discipline with a Minimum of 2 years of Case Management experience in the community.
